Directions
Develop professional speaker notes for the in-person Practicum Project: Health Promotion Collaboration meeting with a minimum of 3 licensed healthcare professionals, following the steps below.
- Download the Week 4 Practicum Project: Speaker Note template.. Use of the template is required. If the template is not used, a 10% deduction will be applied. See the rubric. Save the template and include your name in the file name.
- Use information from Weeks 1 – 3 Practicum Project discussions and assignment to complete the Week 4 Practicum Project: Speaker Note Template.
- Correctly cite and reference ideas and information that come from scholarly literature. Sources should be no more than five years old unless they are historical references.
- Follow rules of grammar, spelling, word usage, and punctuation consistent with formal, scientific writing.
- Abide by Chamberlain University’s academic integrity policy.
Include the following sections (detailed criteria listed below and in the grading rubric). Answer the questions under each section with explanation and detail. Information from previous weeks may be used.

- Epidemiological Data: In one or more paragraphs, compare the county, state, national, and world prevalence and mortality data. Discuss how data are similar and how they are different. Explain if the community health concern is improving or worsening. Provide a citation for at least one scholarly source.
- Community and Environmental Assessment Data: In one or more paragraphs, discuss the findings and trends in your community and environment assessment and how they relate to your practicum topic. Examine findings for evidence of the social determinants of health (SDOH). Identify two strengths and two areas for improvement within the community. Provide a citation for at least one scholarly source.
- Educational Material: In one or more paragraphs, discuss your educational material, how this material is appropriate for the community, and how it will help licensed healthcare professionals and community members improve health. Provide a citation for at least one scholarly source.

My name is XXX, an RN-to-MSN student at Chamberlain University. I major in mental health nursing and currently practice at a nursing home in Davenport, Florida in a mental health ward. Health promotion collaboration meetings: These are meetings organized with health promotion professionals from different health fields to discuss matters concerning health promotion in the community, exchange, and plan for the promotion of health normally in the community. Also, it is our vision to develop a holistic community health promotion intervention model by engaging multiprofessional experts and drawing on multiple resources.

My chosen topic is an awareness of mental health. The two biggest reasons many patients may pursue careers in mental health are charitable objectives and personal improvement.
